**Description:**
Medical Social Worker I position accepts the responsibility and accountability for addressing psychosocial, financial, emotional, and environmental factors as they relate to patient health and discharge planning needs. This includes, but is not limited to, Advance Directive issues, patient/family advocacy and coordination of community resources, while maintaining professional social work standards.
**ESSENTIAL FUNCTIONS**

+ Conforms to the National Association of Social Workers (NASW) Code of Ethics
+ Assists patients and families with psychosocial needs including counseling as needed and coordination of family meetings
+ Recognizes problems, systematically gathers data, identifies/understands underlying issues, synthesizes complex issues, seeks input from others and makes difficult decisions to formulate appropriate treatment plans.
