Responsibilities:

• Support Medical Social Workers (MSW) team in direct patient work, administrative work, and general duties.
• Manage routine cases assigned by MSWs independently.
• Provide training for new social work and administrative assistants.
• Conduct financial assessment on eligibility of applicants for the various financial schemes.
• Conduct field visits and facilitate discharges by accompanying patients home or to social service organizations.
• Take on inpatient and outpatient casework independently, conduct follow‑up for discharged patients, and network with community services to facilitate seamless care.
