**Music Therapist - Hospice – per diem Capital/Saratoga region**

Community Hospice is looking for a qualified Music Practitioner/Therapist to join our team! The Music Therapist will provide complimentary therapy services to all levels of care (hospitals, nursing homes, and patients' homes).

The Music Therapist is an important member of our health care team, serving Hospice patients in Home Care, Nursing Home or Inpatient settings

**Responsibilities:**

+ Responsible for direct care and evaluation of patients, with resultant formulation and implementation of treatment program as appropriate to the specific needs of that patient
