As a Licensed Practical Nurse, you will coordinate appropriate medical care, dental care, and mental health services to adult clients enrolled in the Assertive Community Treatment (ACT) program. ACT is a program that provides services through less traditional service delivery methods. This is very much a meet the client in their community approach. You will support the RN and the Psychiatric Nurse Practitioner on the ACT team and be responsible for meeting with clients and aiding in their medication administration and additional needs. You will report to the Director of the ACT Program, a Licensed Mental Health Professional and work onsite and in the community.
