About the Role
We are looking for a motivated and organized Case Coordinator with a Nurse background to join our growing allied health service. You will work closely with an experienced Australian allied health team , supporting high-quality, client-centered services under the Australian Aged Care framework , while being based locally.
This non-clinical role is ideal for someone who enjoys case coordination, client engagement, and collaborative teamwork , and who wants exposure to international healthcare standards .
Key Responsibilities
Be the main contact person for clients, families, and carers
Help new clients get started by collecting basic personal and support information
Arrange and coordinate services with allied health professionals (OT, Physiotherapy, Speech Therapy, etc.)
Assist with putting care and wellbeing plans into place and reviewing them over time
Check on client wellbeing, identify any concerns, and report issues when needed
