Opportunity Description
Become a pivotal member of the CMHA Peel Dufferin as a Nurse Practitioner in the HART Hub, focusing on mental health and addiction recovery services. Work part-time in Brampton.
In this responsible role, you will provide 18.75 hours of client-focused care, attending to addiction and mental health patients both in the clinic and through outreach. Collaborating closely with the RAAM team, you will assess client health, develop treatment strategies, and ensure referrals are made to meet comprehensive needs.
Key Responsibilities:
• Assess and treat clients with addictions and concurrent disorders
• Perform thorough health evaluations and diagnosis
• Work with healthcare providers to coordinate care
• Manage prescription medications within the RN(EC) scope
• Conduct home visits for additional support
Requirements:
