Elevate healthcare access as a Nurse Practitioner with WellFort’s Homeless Health Program. This part-time role focuses on delivering vital services to homeless and precariously housed individuals in the Peel Region.
As an integral part of our healthcare team, you’ll provide comprehensive, integrated care, emphasizing trauma-informed practices and support for mental health and chronic conditions. Your work will directly address the barriers faced by individuals with housing instability, ensuring they receive the care needed in an equitable manner.
Key Responsibilities:
• Conduct detailed health assessments and plan interventions
• Manage medication and therapeutic interventions
• Collaborate with community partners for seamless care
• Provide support in mental health and substance withdrawal
• Maintain accurate EMR documentation and participate in audits
