This role is focused on providing both acute and chronic care management for adult and geriatric patients in our Plymouth and Sheboygan service areas . The Nurse Practitioner or Physician Assistant collaborates with supervising physicians to develop and implement comprehensive treatment plans that improve patient outcomes and quality of life.
Nurse Practitioner/Physician Assistant Key Responsibilities

• Delivers primary care for adult and geriatric patients, addressing both acute and chronic health conditions.
• Develops personalized treatment plans in close coordination with the facility care team and other healthcare providers.
