We are seeking an experienced Nurse Practitioner to join a growing healthcare team providing exceptional care to residents across two senior living communities in the Hudson Valley region.

This position will support residents at facilities located in Middletown and Port Jervis, NY , with travel between both locations.

Schedule:

• Monday – Friday, 9:00 AM – 5:00 PM

• No weekends or on-call requirements

Responsibilities:

• Conduct comprehensive health assessments, physical exams, and medication reviews
