Summary The Marion Illinois, VA Health Care System is seeking a full time Nurse Practitioner to join Primary Care, Women's Heath in Marion, IL. Responsibilities The Women's Health Nurse Practitioner is a licensed independent practitioner with advanced education and national certification who provides evidence-based care for a population of patients. Major Duties and responsibilities: Independently manages a panel of patients with acute and chronic health conditions with an emphasis on Women's Health and sex-related care to include but not limited to health promotion and disease prevention using evidence-based practice. Develops and implements an individualized plan to achieve optimal population health and disease management Demonstrates leadership and works collaboratively with Women's Health Patient Aligned Care Team (PACT) and other interdisciplinary team members.
