- Attend to general health and care of all residents.
- Assist the Social Worker in performing Resident Assessment Form (RAF) for potential residents into the HOME.
- Update colleagues on residents’ medical and nursing matters.
- Advise Head of Home and Deputy Head of Home on admission of potential residents and discharge of residents from medical and nursing viewpoints.
- To conduct RAF for residents every six months or upon request from Head of Home.
- Accompany residents to clinics and hospitals.
- To update colleagues of residents’ upcoming or rescheduled medical appointments.
- Required to be on “standby” for emergency and crisis of residents.
- To conduct and document weekly hypo count tests for residents.
- Update residents’ medical records regularly.
- Top up residents’ medication when necessary, and to prepare medication when residents go on “home leave.”
