About the Role

• Play a key role in helping clients meet their occupational performance goals following hospital discharge.
• Work within a multidisciplinary team to deliver person‑centred care in home and community settings.
• Consistently collaborate, sharing expertise and resources to deliver the best possible outcomes for our clients.
• Play a key role in identifying suitable clients for our service.
