As an OT in this role, you will become an integral teammate in our innovative Take Your Therapist Home Program! What is Take Your Therapist Home? In a world of shorter and shorter SNF stays, Take Your Therapist Home (TYTH) is a one-of-a-kind program where you as the clinician work directly with patients in the SNF and home health providing a continuity of care allowing for you to have a greater impact on patient outcomes! You will be primarily in the home health setting with dedicated time and caseload support for the SNF. You will have a localized coverage area in the surrounding area of the SNF. No more ever-expanding home care coverage areas. Your schedule is based on a traditional work week. Rivetus has been serving the geriatric Post-Acute community since 1998.
