Requirements:
- Consultant with formal sub-specialty training or experience
- License can be General Ophthalmology for all except Pediatric Ophthalmology
- Pediatric Ophthalmology requires Pediatric sub-specialty license
- Surgical Ophthalmologist (not medical only)
- Experience must be in a tertiary care hospital
