  • spinal cord stimulation
  • radio-frequency ablation
  • injections

Requirements

• Board Certified in Pain Medicine
• Licensed in Wisconsin or IMLC
• Certifications: ACLS, BLS
• Credentialing Timeframe: 90 days
• Fellowship required
