Role Overview

One of our valued partners is seeking a full‑time Patient Outreach and Enrollment Specialist that can work from Edge Campuses to provide essential clinical support and population health management. Reporting directly to the Director of Clinical Effectiveness, this position functions as a vital link supporting care teams to ensure patients, providers, and clinics are thoroughly prepared for high‑quality, efficient visits.

The ideal candidate will possess a healthcare educational background and at least one year of healthcare experience in fields such as medical assisting, care coordination, patient outreach, or population health, preferably within a Federally Qualified Health Center (FQHC) environment.
