PCN Social Prescriber Link Worker- Children and Young Persons

Bradford City 5 PCN are seeking a compassionate and proactive Children and Young Persons Social Prescribing Link Workers (SPLW) to join our well established team consisting of Social Prescribing Link Workers (Adults), Health and Wellbeing Coaches and Mental Health Practitioner. Thisrole is central to supporting children, young people and their families/Carers empowering them by connecting them with non-medicalcommunity support.

You will help individuals improve their health and wellbeingby addressing wider social determinants and enabling them to focus on whatmatters to them.

This is a person-centred role, working with children and young people fromdiverse backgrounds, including those who may be socially isolated, living withlong-term conditions, or facing complex social challenges.

This is a full-time, practice-based role, but applicationsfrom those wishing to work part time will be considered.
