Provide transportation and peer support to treatment services for clients with opioid use and substance use disorder.

 Collaborate with multi-disciplinary team to provide recovery support services.

 This role will require driving to various community-based stakeholders across Long Island and other boroughs.

 Field-based and on-site peer support to prospective clients.

 Assist client with person-centered recovery goal planning.

 Collaborate with client and care management team to identify the services clients should receive based on stage of change, immediate needs, and recommended treatment objectives.

 Provide services to clients to address housing, income support, education and vocational training, social supports, employment, and primary care needs.
