Per Diem Hospice Nurse

Travel is required for this position. Care is provided in patient’s private homes and residential fac ilities throu ghout Greater Boston and parts of Metrowest
Job Responsibilities :
+ Participate as a member of the interdisciplinary team by providing clinical assessments of patients’ ADLs, and patient’s functional, psychological, and environmental needs. Implements and monitors aspects of patient care including managing discomfort and symptom relief.
+ Utilizes specialized nursing skills and agency resources related to end-of-life care in accordance with the Nursing Care Coordinator’s plan of care and in accord with Good Shepherd’s policies...
