Epic Nursing Services is seeking a compassionate and dependable RN to complete monthly follow-up visits for our AFC and GAFC members throughout Massachusetts. - Conduct monthly follow-up visits with members
- Monitor and document changes in health condition
- Follow up with PCPs, managed care plans, and other providers as needed
- Help coordinate and confirm necessary appointments
- Maintain clear, accurate, and timely documentation
- Communicate effectively with agency staff to support continuity and quality of care
