The individual informs that the arrangement of 2 visits

Tuesdays and Thursdays (the days he does not attend the day centre) , as it is giving him things to do and opportunities to socialise with people and engage in his community.

PA to ensure the Individual is taken out around his care support to ensure medication and nutritional needs are met.

PA to ensure the Individual has his wheelchair
