As a member of the team, a primary function will be to provide short‑term supports to individuals returning from hospital or to prevent admission, in addition to other aspects of service delivery such as assisting individuals who have fallen but are uninjured and responding to telecare alerts when individuals activate their pendant alarm or when no movement is detected in their home. This is a highly rewarding and varied role where no two days are the same; you should enjoy working with people, demonstrate empathy, and be committed to delivering high‑quality care within the community.
