Physiotherapist - Home Care

Responsibilities: Report to the Home Care Management team. Be part of the management team in overseeing the care of patients with direct responsibility of rehabilitation services. Doing home visits to assess, evaluate, identify rehabilitation needs and provide necessary treatments of clients for home therapy. Assist the Rehab Manager with the Centre-Based rehabilitation services when required. Evaluate and monitor progress of patients' treatment regularly. Work closely in a multi-disciplinary team to create Individualised Care Plans for each patient from admission to discharge planning. Ensure strict compliance to MOH regulations and other governing bodies in the performance of services. Assist to maintain database of all clients, ensuring strict compliance to documentation, providing evidence-based treatment with accurate and timely records.
