We are seeking an enthusiastic and motivated Registered Nurse (Band 7) to join our forward-thinking Pre-assessment and Population Health Management Team at West Middlesex Hospital.

This is a unique and exciting opportunity to establish and develop the Trust's first Population Health Management Clinic , while also playing a key leadership role within the Pre-assessment (POA) service . The post holder will work collaboratively with hospital and community teams to deliver a high-quality, patient-centred service aimed at improving both individual surgical readiness and wider population health outcomes .
