Practice Overview

• You’ll guide and mentor a primary care team that includes nurse practitioners and a strong interdisciplinary support staff
• Patient-centered medical model integrating primary care, behavioral health, and dental services
• Dedicated support staff, including triage nurses, a population health manager, a community health worker, a referral specialist, and one medical assistant per provider
• Shared phone call rotation of 1:6 with minimal call volume
