Primary Care Physician - PACE/Geriatrics

About the Opportunity

We are seeking a Primary Care Physician to join a mission-driven, value-based care model focused on helping vulnerable seniors remain safely in their homes and communities.

This is a strong fit for a physician who wants to build deeper, longer-term relationships with patients instead of rushing through high-volume visits. You will work closely with an interdisciplinary team to provide comprehensive, coordinated care for seniors with complex medical, social, and functional needs.

This role is especially well suited for a physician who has experience working within an interdisciplinary team structure and is comfortable partnering with, supporting, or leading Nurse Practitioners and Physician Assistants.
