Flemingdon Health Centre is expanding its team of Physicians to provide comprehensive primary health care services throughout the East Toronto area. This position emphasizes collaboration within interdisciplinary teams to improve client outcomes while focusing on social determinants of health. You will play a vital role in delivering holistic care and engaging with various community resources.
Key Responsibilities:
• Deliver direct client care following established standards
• Perform diagnoses and develop treatment plans
• Engage in community health education efforts
• Make referrals and conduct home visits as needed
• Collaborate with multidisciplinary health teams
Requirements:
