Duties
- Work as part of a team and understand the strengths and challenges of multi-disciplinary working and be able to deliver an effective and efficient service to the public.
- Safeguard and promote the wellbeing of adults through the provision of high-quality social work practice in assessment, care planning, and review.
- Co‑produce and implement care and treatment plans and act as a care co‑ordinator in accordance with key legislative requirements of the Mental Health Measures Wales (2010).
- Work within and understand the principles of the Social Services and Wellbeing (Wales) Act (2014).
- Undertake mental capacity assessments and work within the principles of the Mental Capacity Act 2005.
- Be familiar with WARRN risk assessments and assess risk in a collaborative and structured way that balances risk with autonomy and choice.
