Key Responsibilities
- Conducting comprehensive assessments of clients’ needs and circumstances.
- Developing and implementing person-centered care plans in collaboration with clients, families, and other professionals.
- Providing ongoing support, advice, and advocacy to clients and their families.
- Working closely with multidisciplinary teams to ensure integrated service delivery.
