Summary This Quality Management Consultant Registered Nurse position is located in The Directors Office at the VAMC West Haven, CT. This position is a full time position at 40 hours per week. Responsibilities Included but not Limited to: Delivers fundamental knowledge-based care to the program while developing technical skills. Assesses, plans, implements, and evaluates delivery of care based on age specific components. Demonstrates performance and leadership that is broad enough to improve the care for a group of patients. They collaborate with members of the interdisciplinary team and leading to process improvement initiatives. Assumes responsibility for the coordination of care focused on patient education, self-management, and customer satisfaction throughout the continuum of care. Follows procedures per established policies and guidelines.
