Seeking a compassionate and adaptable Registered Nurse (RN) to join their multidisciplinary healthcare team in an outpatient primary care/FQHC setting. This role focuses on delivering patient-centered care, case management, patient advocacy, and health education to underserved and high-risk patient populations, including individuals with complex medical and social needs. The ideal candidate thrives in fast-paced environments, works collaboratively across care teams, and is passionate about improving healthcare access and patient outcomes within the community.
