Registered Nurse- Community Care

Summary The Community Care Registered Nurse (RN) functions directly to support the activities of the St. Cloud VA Health Care System Community Care. The RN is responsible for the planning, integrating, implementing, modifying and evaluating the effectiveness of the Veteran's care. The Community Care foundational elements will serve to support delivery of high quality, timely care to the Veteran receiving services. Responsibilities Major duties include, but are not limited to: Assesses clinical appropriateness and appropriate level and access to care, reviews and process referrals timely within scope of practice. Demonstrates effective communication skills and communicates plan of care effectively while monitoring transitions of Veteran safely within their scope of practice throughout the EOC.
