The Continuum of Care Department at St Vincent’s Private Hospital Sydney (SVPHS) provides a coordinated, patient-centred service that supports safe, efficient care across the patient journey, with a primary focus on preadmission assessment and discharge planning.

The department undertakes comprehensive preadmission screening and planning, ensuring that patients are appropriately prepared for admission.
