Registered Nurse (Inpatient Case Management)

Summary VA Greater Los Angeles Healthcare System (VAGLAHS) is seeking to hire an experienced Registered Nurse to work in our Inpatient Case Management Service. Incumbent is expected to function independently, collaboratively, showing leadership and communicating effectively, coordinating care for complex/high risk patients, for a seamless, efficient and safe discharge. Responsibilities Current, permanent GLA employees will be given the first consideration. Must submit most recent SF-50 to receive 1st consideration. Job duties include, but are not limited to, the following: Conducts comprehensive patient assessments. Conducts comprehensive risk assessments to identify high-risk Veterans and collaborates with the care team to create personalized care plans that mitigate harm. Coordinates complex care.
