Registered Nurse - Interventionist (Calgary)

Position Overview

Integrated Home Care uses a strengths-based client-centered case management practice framework. Using a person-centered approach, you will develop a care plan supporting the client/family to identify needs, focus on strengths, set achievable personal health and self-care goals, and provide direct nursing care.

In collaboration with the client/family you will ensure services are provided and modified on an ongoing basis, based on the client’s response to interventions, incorporating stewardship into service authorization. You will report to the Care Manager and be responsible for coordinating and providing quality care to clients in the community. You will provide nursing assessments, case coordination and direct care in partnership with clients/families in their own residence or an alternate care setting, as determined by the assessment.
