SUMMARY:
The Registered Nurse in home health provides one on one skilled nursing care and treatment to patients in their homes using a comprehensive and patient specific care plan. This position will have the opportunity to spend quality time with the patient, in the patient's home, to promote health and independence; have autonomy in addition to the support and collaboration of a full team of healthcare professionals while managing a case load; maintain compliance with applicable laws, regulations and agency policies; and provide accurate documentation and follow-up of patient findings to provide optimal patient outcomes.
