Acute Care Case Manager – Care Coordination

Weekend rotation required; specific weekend shifts vary based on unit needs.

Minimum 2 years of acute medical care management experience with care coordination. Travel experience required (no first-time travelers). California license required (pending accepted during process but must be active on start date). Epic experience is required.

Conduct patient assessments and coordinate care for adults and geriatrics. Develop and implement discharge plans and participate in family meetings. Arrange durable medical equipment (DME), skilled nursing facility (SNF) placement, and transport as needed. Manage complex psychosocial needs, including homelessness, mental health, and substance use. Ensure compliance with CMS, DRG, and JCAHO standards, along with HIPAA and OSHA regulations.
