**RN HOSPICE-Care Manager for Nursing Homes in the Schenectady and Albany county regions primarily.**

The Community Hospice has a wonderful opportunity for a FT RN to work as part of our Hospice Care Team caring for patients and families in local Nursing Homes.

Monday - Friday 8a-4:30p
