Registered Nurse (Rn) - Home Health

We are partnering with a growing home health organization seeking an experienced and compassionate Registered Nurse (RN) to join their team full-time. This position is ideal for an RN who enjoys autonomy, case management, and building strong relationships with patients in the home setting.

The RN will provide skilled nursing care to patients throughout the south Denver metro area, including territories extending south of I-70, west toward Golden, east toward Aurora, and beyond Littleton as needed.

This role includes a mix of:

• Start of care visits
• Routine skilled nursing visits
• Recertifications
• Discharges
• Ongoing case management

The schedule is Monday through Friday with daytime hours.

• Medication management
• Wound care
• Coordination with therapy services
• Patient education and care planning
