Summary The Care Coordination Integrated Case Management (CCICM) RN Co-Coordinator is an administrative position that works in partnership with the CCICM Social Work Co-Coordinator to effectively ensure that facility nurses and Social Workers provide case management and care coordinator adhering to the CCICM framework. Responsibilities List of major duties: The VA medical facility CCICM Nurse Coordinator and Social Work Coordinator serve as the VA medical facility CCICM Co-Coordinators.
