Remote Cardiology Patient Navigator

Seamless Assist is looking for a Cardiology Patient Navigator (Coordinator) to coordinate patient care for a U.S.-based cardiology team.
You will manage patient communications, scheduling, and clinical documentation to ensure continuity of care for over 150 patients.The ideal candidate has a clinical background, experience in telehealth, and is bilingual in English and Spanish.
This remote position requires comfort in managing multiple priorities while providing exceptional patient-centered care.
