Transfer Pump-01 experienced repeated tripping during operation. Troubleshooting and inspection revealed that the associated circuit breaker (Merlin Gerin NBD250H, 250A, 3-Phase) was supplying only two phases at the output. The R phase was absent, while the Y and B phases remained available.

A detailed examination of the circuit breaker identified failure of the internal contacts and deterioration of the operating mechanism. The defective condition prevented proper engagement and continuity of the R phase, resulting in a two-phase supply condition to Transfer Pump-01. The failure was primarily attributed to aging of breaker components and mechanical malfunction of the operating mechanism.
