Resident Care Social Worker — Plan, Coordinate & Support

THE SUPREME HR ADVISORY PTE. LTD. is seeking a Social Worker to develop and execute individualised care plans for residents. The role involves coordinating care with various stakeholders and monitoring progress.

A degree in Social Work is required, and experience in the social service sector is preferred. The position is located in Bukit Batok with working hours from Monday to Friday, with occasional Saturday work.
