Cleveland Clinic Care Coordinators have been very successful in helping patients manage their own care. Their hard work, dedication and commitment has led to a decrease in Emergency Department visits, observation status, inpatient stays and hospital readmission in care coordinated patients. As a Care Coordinator, you will support a new physician's orthopaedic practice.

**A caregiver in this position works between 7:30am and 5:00pm, however most days will be from 8:00am to 4:30pm. No weekends or holidays required.**
