RN Case Manager - DWC

**Full-Time, Regular grant-funded position at the DWC. This is under the Bristol Meyers Grant that works on low-dose CTs for cancer prevention | Schedule: Mon-Fri 8a-4:30p | Pay: Paygrade 13, Hourly, Dependent upon experience**

**Job Purpose or Objective(s):** The Case Manager RN provides nursing navigation support for individuals participating in the lung screening program and promotes continuity of care throughout the screening process within the Diabetes Wellness CenterLung Cancer Screening Services.
