RN Referral Coordinator

PRN – As Needed

Normal Business Hours – Monday through Friday 8 am to 5 pm

Must be able to work 1-2 days a week

Ability to support IL team needs

Coordinating with multiple hospital locations in IL

Supporting both hospice and home health needs!

Hospital experience is a plus!
