Senior Geriatrician: Frailty & Hospital-at-Home Lead

CDDFT Charity is seeking a Consultant Geriatrician to lead the assessment and management of frail patients. This consultant-led role is part of an integrated model aimed at improving patient outcomes and reducing hospital admissions.

The position will involve working closely with a multidisciplinary team in the Frailty SDEC and supporting the Hospital at Home service. Emphasis is placed on education, collaboration, and maintaining high clinical standards.

Salary ranges from £113,565 to £150,569 per annum with a commitment to professional development.
