Job Summary

As a member of the multi-disciplinary Dementia Outreach Team within Nottingham CityCare Partnerships Integrated Care Homes Service (ICHS), you will undertake specialist assessments of care home residents living with dementia. You will develop and implement treatment plans, using clinical knowledge to enhance physical and social environments, supporting individuals to engage in activities of daily living.

Job Terms

This is a 12 month fixed-term role working 22.5 hours per week.

Responsibilities

• Perform holistic patient assessments and develop individual therapy programs.
• Manage a clinical caseload of care home residents with dementia.
• Collaborate with team members, specialist services, and physical therapy clinicians within the integrated therapy team.
