Senior SEND Transition Coordinator

Guardian Jobs is looking for a Senior SEND Transition Worker based in Uxbridge / Hayes. In this full-time and permanent role, you will support young people with Special Educational Needs and Disabilities (SEND) during their transition to college.

Your responsibilities will include overseeing the administrative processes for learners with an Education, Health and Care Plan (EHCP) and maintaining communications with local authorities. Key skills include experience in the education sector and strong IT capabilities.
