Responsibilities
- Conduct comprehensive biopsychosocial assessments for patients, residents, clients, and caregivers, and implement appropriate interventions in line with established social work professional standards.
- Manage the full spectrum of social work services, including assessment, care coordination, intervention planning, counselling, emotional support, monitoring, and outcome evaluation.
- Address clients’ psychosocial and financial concerns, while contributing to discharge and transition planning as part of integrated care management.
- Work collaboratively with healthcare professionals such as physicians, nurses, therapists, and other allied health staff to deliver coordinated and person‑centred care.
- Engage and reach out to individuals and communities who may benefit from group work interventions and support programmes.
